PREPARING FOR YOUR FIRST OBSTETRIC APPOINTMENT

To help you get ready for your first appointment with your Prologue Obstetrician, we have put together this checklist. The entire Prologue team are here to help and make things as easy as possible for you. If you need any assistance in getting these things done, please call the rooms and we will work with you.

  • 1. Complete the online patient registration form

    A link to this form was supplied in the original email you received from the practice. You only need to fill out this form once and those that are already patients of the practice do need to fill it out. If you have any questions regarding this form or whether you have completed it (you will have received a confirmation email if you have), then you can contact us on adminsupport@prologue.com.au

  • 2. Provide a referral from your GP

    A referral from your GP allows us to claim and refund to you, your medicare rebate for each visit. Whilst this referral is preferably supplied by your regular GP, it can be from any GP including one that is bulk billing and/or consults via Telehealth. It must be dated prior to your first appointment with us.

  • 3. Complete a Dating Scan

    It is important that you have had a dating scan before your first appointment with your Prologue Obstetrician. It is usually arranged by your GP and only needs to be done once. If you would prefer, we can provide you with a request form to have your dating scan done. Just reach out to us on pregnancysupport@prologue.com.au

4. Use the pathology request provided by Prologue to have your blood and urine tests done at a DHM collection centre at least one week prior to your first appointment with your Prologue Obstetrician

You will receive an SMS from Prologue that contains a QR code for Douglass Hanly Moir Pathology (DHM). This will link to a pathology form that will allow you to get a set of blood tests that will help us prepare you for your first appointment. You can visit any DHM collection centre to complete the tests. You can see a list of the collection centre locations and their opening times here. There is no need to book, pay, or fast for any of these tests. Simply show them the QR code on your phone when you arrive and they will take it from there.  All of the tests on the form are bulk billed so if you are covered by Medicare, there will be no out-of-pocket cost to you.
